Healthy Play

The Gympie Community Place Inc. Healthy Play Project supports child development and health literacy for marginalised and vulnerable families in Gympie and surrounding locations.

Paediatric Early Intervention Program

The Paediatric Early Intervention Allied Health Service is funded by CCQ to deliver Speech Pathology, Occupational Therapy and Psychology by telehealth to pre-school aged children and families in the Wide Bay and Central Queensland areas.

GP Services & Preventative Women’s Health

The GP Services and Preventative Women’s Health activity is funded to improve access to a female General Practitioner specialising in women’s reproductive and sexual health for women identified as at risk in the target area.

Pulmonary Rehabilitation & Lungs in Action Program

CCQ is funding the Activity to increase access to pulmonary rehabilitation and pulmonary maintenance services on the Fraser Coast.

Rural Primary Health Services

The Rural Primary Health Services (RPHS) Activity aims to improve health outcomes and service access by specifically targeting priority areas and groups with high prevalence of risk behaviours.

QLD Virtual Integrated Practice GP

The aim of this Program is to supplement the General Practitioner (GP) workforce in eligible rural catchments (MMM 4-7) and increase access to local primary healthcare.

Little Beginnings

The Little Beginnings Program aims to increase access to antenatal and postnatal education to support optimal maternal and infant health and support positive health behaviours.

Foundations for Life

The Foundations for Life program aim to improve the health and wellbeing outcomes for pregnant women aged under 25 years and their infants who are at greater risk of poor health outcomes, particularly Aboriginal and Torres Strait Islander women.

Mums and Bubs Program

The Mums and Bubs program aims to enhance the health and wellbeing of Aboriginal and Torres Strait Islander mothers and their infants in the Fraser Coast LGA, particularly those who are at greater risk of poor health outcomes.

Community Carers and Responders (CCR) model

Drawing on the successful implementation of the Community Carers and Responders (CCR) model implemented in Northern Rivers NSW since 2021, Safer Future in partnership with Plan C, is implementing the CCR Network.

Caring Connection

As a generalist first-stop community support agency, Waves of Kindness (WOK) aims to empower individuals to build connections and confidence for improved wellbeing and resilience.

Gympie and District Women’s Health Group (GWHC)

The Gympie and District Women’s Health Group (GWHC) is using the Flood Wellbeing and Resilience Grant to enhance the mental health and wellbeing of women in rural Gympie.

Migrant Development Program

The Maryborough Neighbourhood Centre aims to strengthen social ties and educate the Culturally and Linguistically Diverse (CALD) community about disaster readiness and resilience through its ongoing Migrant Development Program (MDP).

Youturn garden

Youturn, run by headspace Gympie, is creating an outdoor garden to boost social connections and resilience. With funding from the Flood Wellbeing and Resilience grant, they aim to improve the mental health and resilience of 400 young people.

Mob Pod

The North Coast Aboriginal Corporation for Community Health has launched the “Mob Pod” health initiative to improve the health and wellbeing of Aboriginal and Torres Strait Islander people in Gympie and Noosa’s rural areas.

Neuroscientific education and training

The Thompson Institute is providing neuroscientific education and training to disaster management professionals, volunteers, and community members in areas affected by flooding, specifically in Noosa, Gympie and Maryborough.

Resilient CARE project

Aims to reduce the negative health impacts of both natural disasters and a changing climate in five local government areas across Wide Bay and Central Queensland.

Geriatric and cognitive impairment outreach service

This activity aims to deliver a regionally based, networked geriatric and cognitive impairment outreach service providing both direct clinical care and capability-building.

Virtual Care Pod

The CCQ Care Pod is a purposefully designed pod to provide a safe, private space to connect to a confidential video consultation with a virtual doctor (GP).

Nurse practitioner model (RACHs)

This activity enables the delivery of Nurse Practitioner (NP) services within 2x Residential Aged Care Homes in Gympie and Banana (Biloela) LGAs.

Integrated Team Care

Integrated Team Care is a program delivered by a team of Aboriginal and Torres Strait Islander Outreach Workers and Care Coordinators across the Indigenous and mainstream primary care sectors.

First Nations Mental Health, Alcohol and Other Drug and Social and Emotional Wellbeing

Across Central Queensland, Wide Bay and the Sunshine Coast, a range of local Aboriginal and Torres Strait Islander mental health and alcohol and other drug services and social and emotional wellbeing services are available.

Care finders program

Care finders can help vulnerable older people navigate the aged care system and find support services to improve their quality of life.

ZERO Homeless Health Initiative

The CCQ ZERO Homeless Health Initiative, led by IFYS and Roseberry Queensland, applies the Advance to Zero methodology to unite services through shared data and coordinated care, addressing system gaps and improving health, housing and homelessness.

PHASES with Primary Sense

PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) is a statewide collaboration of the 7 Queensland Primary Health Networks, Queensland Health and the Australian Government Department of Health, Disability and Ageing.

Better Connect is a coordinated network of free mental health, alcohol and other drug, suicide prevention, and psychosocial support services across Central Queensland, Wide Bay and the Sunshine Coast.

Family and Friends Program

Medicare Mental Health Centres

Medicare Mental Health Centres offer free, safe and welcoming services for adults over 18 who are experiencing distress, feeling overwhelmed, or concerned about their mental health or someone they care about.

Medicare Urgent Care Clinics

Medicare UCCS provide short-term, episodic care for urgent conditions that are not immediately life threatening. Medicare UCCs must be partnered with an accredited general practice.
