Opportunity Country to Coast Queensland (CCQ) has leased new office premises at 62a Barolin Street, Bundaberg South, to replace its existing Bundaberg office. The new premises will provide a contemporary, fit-for-purpose workplace that supports staff wellbeing, collaboration and operational requirements while aligning with CCQ's workplace standards established across its regional offices. The premises require refurbishment and fit-out works to enable the site to be used as a fully operational office. The works include office and work spaces, meeting facilities, staff amenities, communications infrastructure, building services upgrades and associated external works. Through this procurement, CCQ seeks to engage a suitably qualified and experienced contractor to deliver the Bundaberg office refurbishment and fit-out works.

Opportunity Country to Coast Queensland (CCQ) invites general practices in the Gympie, North Burnett, and Livingstone LGAs to apply for the co-design and trial of innovative Multidisciplinary Team (MDT) Primary Health Models. One general practice will be selected per LGA to partner in this initiative.
Purpose This opportunity aims to strengthen chronic disease management, enhance patient care coordination, and build the capacity of the local primary healthcare workforce. The initiative responds to the complex needs of individuals living with chronic conditions, addressing physical and mental health, social support, transport, and financial challenges.
Funding and Term

General practices are required to enter into an Agreement for a two-year term. The following funding amounts have been allocated per general practice (one practice per LGA):

  • FY25–26: $96,000 (January to June)
  • FY26–27: $195,333
  • FY27–28: $96,000 (July to December)
Commissioning approach Successful applicants will participate in a co-design session facilitated by CCQ to finalise the MDT model, agree on key performance indicators (KPIs), and establish reporting requirements. This commissioning method reflects CCQ’s commitment to system reform, community consultation, innovation, and flexibility while addressing local service gaps and promoting equity in service design and delivery.
