Why these resources matter
Dementia is now the leading cause of death in Australia. In 2026, an estimated 446,5001 Australians are living with dementia, including around 88,2001 people in Queensland.
A dementia diagnosis can bring change, uncertainty and many practical decisions. Having important information in one place can make it easier for people living with dementia, families, carers and care providers to stay connected and informed.
In partnership with the Australian Government Department of Health, Disability and Ageing and Dementia Australia, Country to Coast Queensland has developed practical journal resources to support conversations, care planning and shared understanding.
