This activity enables the delivery of Nurse Practitioner (NP) services within 2x Residential Aged Care Homes in Gympie and Banana (Biloela) LGAs, as areas which have been identified as having significant primary care access challenges. The primary goal of the service is to meet the clinical needs of residents within these homes. Additional objectives include enhancing collaboration and relationships across the multidisciplinary care team, uplifting the sectors’ understanding of the NP role within the aged care setting, providing capacity building to staff within RACHs through education and mentorship, and supporting the utilisation and integration of digital health initiatives for streamlined care.
