PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) is a statewide collaboration of the 7 Queensland Primary Health Networks, Queensland Health and the Australian Government Department of Health, Disability and Ageing.
It aims to deliver a proactive approach to identification, early intervention and management of cardiovascular disease in primary care. By leveraging the data analytics capability of Primary Sense, prevention efforts are strengthened, enabling a comprehensive review of patient needs at the point of care and giving general practice the opportunity to offer proactive guidance on lifestyle and behaviour modification alongside medication options.
Improving the treatment of cardiovascular disease in primary care will subsequently improve long term health outcomes and reduce the demand and cost to Queensland’s hospital and health services.
