IMPACT Resilience – Discovery Coast Primary Health Care Continuity Plan

Part of the Resilient CARE Program 2025 grants.

The IMPACT Resilience project brings together local primary healthcare providers to ensure consistent care before, during, and after disasters. It focuses on developing an integrated Primary Health Care Continuity Plan for the Discovery Coast region, which will be activated in alignment with the Gladstone Regional Council Local Disaster Management Plan.

The plan will:

  • Strengthen coordination between primary healthcare providers and external agencies.
  • Enhance the capacity of general practices to identify and support high-risk patients.
  • Improve community resilience and ensure continuity of care when the region becomes isolated during natural disasters.

Currently, no such coordinated health mechanism exists for the Discovery Coast, making this initiative a vital step toward a more resilient and connected healthcare system.
