The Resilient Ready Program supports primary care teams to strengthen community preparedness for emergencies, with a focus on older people living independently.
Through home visit assessments, personalised disaster plans, and the distribution of Resilient Ready Kits, the program helps individuals build practical resilience before disaster strikes.
At the same time, it enhances practice systems to identify and monitor at-risk patients and supports better coordination with local disaster management efforts. By linking older people living independently with existing dashboards and council disaster plans, the program improves communication and response during emergencies.
The project also generates insights to guide similar resilience initiatives across other practices and regions.
