My Health Record: The right information, right when it matters 

Australia’s digital health landscape continues to evolve, and for primary care providers, My Health Record remains one of the most practical tools available to support safer, more connected care. For general practice and aged care providers, this is particularly valuable for people with chronic and complex health needs who regularly move between services.  

Access to a shared, national health record can support safer transitions of care, reduce duplication, and improve coordination between GPs, hospitals, pharmacies, allied health and residential aged care services.
