GP Smart referrals

Smart Referrals replace clunky, paper-based processes with fast, accurate, digital workflows that saves time for practices and improves patient experience. Integrated with GP software, GP Smart Referrals ensures the information sent is complete, secure, and trackable. For patients, that means fewer delays and access to care sooner.  At CCQ, we support GPs to adopt Smart Referrals as part of a modern referral pathway that enables a more connected digital health system that benefits clinicians, patients and the wider community.

GP Smart Referrals is a digital referrals capability enabling faster, streamlined management of referrals to and within Queensland public hospitals.

GP Smart Referrals utilises information extracted from the patient’s record in the clinical software in use during the consultation. SNOMED CT-coded information is held for specialtiesand conditions. The completeness and accuracy of the coded information in the clinical software is important as it determines the performance of GP Smart Referrals.

This user guide provides an overview of the functionality within the GP Smart Referrals application and the various components within this decision support tool.

This document is intended to assist users of the GP Smart Referrals product. GP Smart Referrals has in-built referral management capability designed to have a more streamlined approach for healthcare professionals, and, to improve the quality of referrals.
