People living with disability bring unique perspectives, talents and strengths that enrich our communities, and yet consistently experience higher rates of poor health and often face barriers in accessing the primary healthcare system, such as physical inaccessibility, lack of inclusive communication, and services that don’t always meet their needs. 

We understand that people with disability are experts in their own lives, and their voices, choices and lived experiences must be at the centre of shaping primary health care. We work alongside disability advocates, primary care providers and community partners to ensure health systems are inclusive, services are accessible, and care is built around the person. Our focus is on creating the conditions that enable people with disability to access the right care, in the right place, at the right time.
