healthy ageing and end-of-life care

Older people are valued members of our communities, contributing wisdom, care and leadership. As people age, they should be empowered to live well, stay connected and receive coordinated care when it’s needed most. We support residents and staff in aged care homes, partner across sectors to strengthen quality of care and promote healthy ageing in the community. Our work includes improving referral pathways, supporting age-friendly communities and ensuring compassionate, dignified end-of-life care.

Healthy Ageing Strategy 2022–2027​

Our regional strategy sets a shared vision: people are valued as they age, empowered in their choices and supported by progressive, sustainable systems.

Healthy Ageing coordinators

Our coordinators partner with residential aged care staff, GPs and nurse practitioners to connect services, organise in-reach and support practical improvements that make care safer and easier.
