The right care in the right place — 24/7
Nearly 10,000 people live in residential aged care across Central Queensland, Wide Bay and the Sunshine Coast. We know that around one in four hospital transfers from Residential Aged Care Homes (RACHs) could be avoidable.
In partnership with aged care, the Queensland Ambulance Service (QAS) and local Hospital and Health Services, CCQ has developed the Connected Care RACH Toolkit — a suite of co-designed resources to help staff make confident decisions about care pathways, coordinate with health partners and keep residents well-supported in their home.
These tools support, but do not replace, clinical judgement.

CCQ has developed this flyer and webpage to outline how RACH staff, supported by a network of medical professionals, provide round-the-clock care with a focus on treatment in the comfort of home whenever possible.

Why These Resources Matter
In 2024, CCQ held a series of workshops with RACH staff, QAS and HHS through our regional Aged Care Collectives. Four consistent reasons emerged for hospital transfers that may not always have been necessary:
- Family request
- Resident request
- Lack of GP access or medical assistance after hours
- Staff lack of confidence and/or skills to manage the situation
The Connected Care RACH Toolkit was co-designed to directly address these challenges — improving after-hours support, building staff confidence, reassuring families and strengthening collaboration across the system.
Impact So Far
- 77% of after-hours calls to the GP FastTrack line have resulted in residents being treated in place (as of May 2025).
- All 101 residential aged care homes across our region will receive the toolkit.
- Homes and health services report improved communication and more timely in-reach support to keep residents at home whenever possible.
