Free, practical resources for people living with dementia, their families and carers. Developed with the Australian Government Department of Health, Disability and Aged Care and Dementia Australia, our Living well with Dementia – My Journal and matching fridge magnet make it easier to share information, plan care and keep key contacts on hand.

Dementia is now the leading cause of death for Australians, according to the Australian Institute of Health and Welfare (AIHW). In 2025 there are an estimated 433,300 Australians living with dementia.

Managing a diagnosis and the daily complexities of dementia can be challenging. In partnership with the Australian Government’s Department of Health, Disability and Aged Care and Dementia Australia, Country to Coast Queensland has developed resources to support people living with dementia, their families and carers.
