Country to Coast Queensland is supporting primary care teams to provide more inclusive care for people with intellectual disability.
This work is being delivered through the Primary Care Enhancement Program, part of the Australian Government’s National Roadmap for Improving the Health of People with Intellectual Disability.
Around 450,000 people in Australia live with intellectual disability.
Many experience poorer health outcomes, including higher rates of chronic disease, mental ill health and avoidable health issues.
The Healthy Together Project aims to help by providing practical tools and training for primary care teams.
e•learning modules
Primary care professionals can access five CPD-accredited eLearning modules.
The modules are designed for GPs, practice nurses, allied health professionals and pharmacists.
They were developed with Parent to Parent and Loud and Clear QLD.
Each module takes around 30 to 60 minutes and includes practical scenarios, tools and resources.
The modules are accredited with RACGP, APNA and APC.
Quality improvement toolkit
Practices can also access the Intellectual Disability Care Quality Improvement Toolkit.
The toolkit helps teams identify gaps, apply evidence-based improvements and strengthen care over time.
It is designed to support more inclusive, person-centred care in everyday practice.
