About the role
City Heart Medical is currently looking for a full-time Registered Nurse to work with our dedicated team of nurses, staff, and doctors providing the highest quality care in the role of Chronic Disease Management Nurse.
In this role, you will be expected to work both autonomously and collaboratively to deliver great patient care. Duties will include all aspects of General Practice nursing with a strong focus on Care Plans and Reviews, Health Assessments, Patient Education and Follow Up etc.
What we’re Looking For:
- You’re a nurse with AHPRA registration and full working rights in Australia (no sponsorship available)
- You’re experienced in GP Chronic Condition Management Plans and Health Assessments
- You’re passionate about patient care
- You hold a current CPR certificate – although assistance can be offered here if it has recently expired.
Remuneration will be based on experience.
