ACP Community Champion Workshop & guide

The advance care planning (ACP) workshop and guide have been developed to support ACP
Community Champions.

Free half-day workshops within the Central Queensland, Wide
Bay, Sunshine Coast Primary Health Network (CQWBSC PHN)
are being offered to community members interested in becoming
ACP Community Champions.
