This June, we break down the latest MyMedicare news, including major changes to Chronic Condition Management MBS items 1 July 2025, GPACI reminders and the new MBS bulk billing incentive program commencing 1 November 2025.

It's time to prepare your practice

Changes to Chronic Condition Management MBS items start 1 July 2025

From 1 July 2025, changes are coming to the way chronic condition care is delivered in general practice, with new Medicare Benefits Schedule (MBS) items to replace current GP Management Plans and Team Care Arrangements.

These updates are designed to simplify care, promote continuity, and improve support for patients with chronic conditions.

While we are still waiting for full details from the Department of Health, Disability and Ageing, Country to Coast QLD (CCQ) is here to help you prepare.

Key changes include:

  • A single GP Chronic Condition Management Plan replacing current GPMP and TCA items.
  • Access to care plans linked to MyMedicare registration for eligible patients.
  • Equal MBS fees for both preparing and reviewing care plans.
  • Simplified referral processes.
  • Transition arrangements in place for two years to ensure current patients maintain access to services.

    What your practice can start doing now:

    1. Engage your patients 
      Start conversations with patients who have existing care plans. Encourage them to register with MyMedicare and return for regular reviews.
    2. Prepare your team 
      Raise awareness of the changes at your next team meeting and consider how existing workflows may need to change to support MyMedicare registration and chronic condition requirements.

    GPACI Latest News

    A reminder: to remain eligible for the General Practice in Aged Care Incentive program (GPACI), you must keep your accreditation expiry up to date. Make sure you check and update with the below useful resources to assist:

    Practices are encouraged to review the updated resources on the MyMedicare Health Professionals Education site, including simulations and updated slide guides, to ensure compliance with these changes.

    The GPACI Toolkit will support primary care practices to utilise the incentive program to address barriers to care delivery, strengthen continuity of care, and implement sustainable changes tailored to the unique needs of aged care settings.

    Bulk Billing Incentive Program expansion

    The new MBS bulk billing incentive program for practices registered with MyMedicare will commence from 1 November 2025.

    The new measure expands eligibility for bulk billing incentive to all Australians with a Medicare card, expanding on the already existing incentive program which is currently limited to children under 16 and Commonwealth Concession Card holders.
