New Patient Travel Subsidy Scheme (PTSS) materials have been developed to improve the way people access PTSS subsidies and information.
The new forms will make it easier for patients and clinicians to apply for PTSS by reducing the duplication of information required for each application. They also compliment the new enterprise IT solution. The three forms in the PTSS process for patients are:
- Patient Registration (Form A) completed by the patient to register for PTSS, and only needs to be filled out once and if the patient’s details need updating.
- Travel Referral (Form B) is filled out by the clinician and includes the patient’s referral as well as details on their travel requirements.
- Appointment Attendance (Form C) completed by the patient’s treating specialist as confirmation the patient attended the appointment.
- Accommodation confirmation (Form D)
To help patients, clinicians and administrators understand the new forms and the information required, user guides with details on how to complete each section have been developed for the three forms. Old forms will still be accepted by HHSs.
To improve the information available on PTSS to all users, a brochure and posters have been developed to raise awareness and understanding of PTSS.
The brochures provide information on the Scheme including details about eligibility and subsidy amounts. To help patients understand the process and manage their expectations of the Scheme, a process map, highlighting key activities in the PTSS patient’s journey has been developed as a key feature. Information on the website will be updated on 1 February 2019 to reflect the changes.
