Living with a chronic condition can dramatically affect an individual’s health and has a significant cost impact on our primary and hospital care system.
In 2023, Australians lost an estimated 4.4 million years of healthy life due to chronic conditions, equivalent to 85% of the total disease burden in that year*.
Earlier detection, diagnosis and referral by primary health clinicians is critical to delaying development and better management of chronic health conditions to reduce potential demand on other parts of the health system.
Help drive an integrated and collaborative approach for better health outcomes for those living with a chronic condition in our region.
