HeartPlan in a box quality improvement (QI) toolkit is intended to support general practitioners and primary care teams in undertaking QI activities to improve the quality of care for their patients with heart failure. It is developed by Novartis Australia with the guidance of a multidisciplinary steering committee of heart failure experts.
