The COVID-19 period has been difficult for patients with type 2 diabetes (T2D) in Australia, with many becoming overdue for their regular review and the proportion of patients with poorly controlled diabetes increasing.
In this webinar, A/Prof Ralph Audehm and another HCP colleague will take you through how their GP practices conduct a best-practice recall and review of their patients with T2D, using a whole-of-practice approach. We will also discuss effective management models involving medication choice based on clinical characteristics and preferences, as well as patient education on self-management and T2D therapies including injectables.
Learning outcomes:
By completing this learning activity, participants will be able to:
- Identify T2D patients due for regular review using patient databases and practice software
- Develop efficient procedures for recalling and reviewing T2D patients utilising the practice team
- Evaluate patients’ current management of T2D to identify any need for management change
- Undertake medication review and patient education on self-management to provide optimal outcomes
Create comprehensive care plans and GPMPs to ensure effective ongoing multidisciplinary care.
