Primary Sense

CCQ is working diligently to improve healthcare for both general practices and their patients. Primary Sense is a population health management and clinical decision support tool that provides real-time clinical decision support for general practitioners at the point of care to give patients the right care at the right time.

Launched in September 2023, CCQ’s Primary Sense Team has hit a tech-tacular mini milestone with 100 installations!

If you’re not in the digital know, you can learn more with Sunshine Coast General Practice, Plaza Medical and Aesthetics in Maroochydore, and their Primary Sense journey, watch video.

129 practices in the Country to Coast, QLD region have now transitioned to Primary Sense – a clinical support, decision making, data extraction and population health management tool.

CCQ is committed to supporting general practice to adopt and make meaningful use of the tool, including to improve patient outcomes and chronic disease management in a primary care setting and at the point of care.

Practices who have installed the software are now able to use Primary Sense prompts and alerts and real-time reporting to provide great care in the clinic room whilst also assisting with accreditation and quality improvement (QI) activities.

In addition to Practice Manager, Radhika from Plaza Medical and Aesthetics, Dr Anthony Rososinski, Birtinya Medical Centre has shared his experience since installing Primary Sense in October 2023 – “The prompt provided are relevant, it helps me identify things that might be missing. This is useful in our busy environment.”

Dr Rososinski, Practice Principal also found the prompts particularly useful in the care planning context, helping the practice to proactively manage complex care needs for their patients. Birtinya Medical Centre use Primary Sense to support quality improvement and plan their clinics. The team works together to identify the clinical areas for quality improvement and leverage the reporting functionality within the Primary Sense desktop to identity at risk patients.

Recently Primary Sense has helped Birtinya Medical Centre to identify patients eligible for health assessments and a bone health clinic for over 70s. Through prompts, alerts and real-time reporting, Primary Sense is helping practices ensure our community have access to timely and high-quality primary care.

Primary Sense allows you to, “Work collaboratively with your colleagues in your practice and together you can build a picture of what will work best for your practice.”
